I, as the parent or legal guardian of the young person above, verify that all information provided is correct to the best of my understanding.

I give my child permission to attend the ARISE Senior Youth getaway and understand that while all duty of care will be taken by leaders to ensure my child’s safety, I waive liability, claims or possible expenses arising out of or in connection with the activities of the getaway. Leaders will not accept responsibility for injury or misadventure over the weekend.

In the event of accident or injury, I give permission for suitably trained leaders and Baden Powell staff to administer first aid if required and understand that I will be notified as soon as possible after the event. I further authorise the use of ambulance and/or anaesthetic by a qualified medical practitioner if in his/her judgement it is necessary. I accept responsibility for payment of all expenses associated with such treatment.

Finally, I acknowledge there are camp rules that my child must adhere to while on camp (including but not limited to prohibited substances, out of bounds areas etc.). If my child doesn't follow these site rules or the directions of Arise leaders, I will come and pick up my child from the campsite if requested by the Arise leadership team.
